Here is my take. Suzuki is gambling that you will need fewer than $400 worth of repairs after one year but before the end of the third year. Obviously, on average they must be making money or they wouldn't offer extended warranties. They think you won't need the repairs. Remember that the extended warranty doesn't cover normal service items, and you have a full warranty for the first year anyhow.
I didn't get one, and so far with 11000 miles I've had no repairs (after 2.5 years). If you ride a lot of miles, it's probably worth it. If you pull the bike out of the garage once a week for a short ride around town, it's probably a waste of your money.
